The DEA will only accept forms that appear as originals.  Follow these steps to have the PDF forms print correctly:

            For Adobe Acrobat Version 5.0; in the Print Dialog, please ensure the following option is selected:

                1.  Auto-rotate and center pages.

            Ensure the following options are not selected:

                1.  Reverse pages

                2.  Print as image

                3.  Print to file

                4.  Shrink oversized pages to paper size

                5.  Expand small pages to paper size
